Have you been on a team that has a "contract?" If so, can you share a copy of the contract?

Some teams have parents sign contracts regarding parent and player behavior and also regarding the fact that their is no refund if a player leaves the team by their own choice.
I do not have a copy of one but no sure what else needs to be agreed to besides the main two points above.
